STATE PENSION shortfalls have affected thousands of pensioners due to systematic errors. Years of "repeated human errors" and technology has led to underpayments of over £1billion. According to a report by the National Audit Office , 134,000 pensioners were underpaid and those affected should receive a payment of £8,900 on average to make up for the money missing.
